What is CIC Under the NHTD Waiver

Community Integration Counseling (CIC) is an individually designed Medicaid counseling documentation service that helps waiver participants manage the emotional challenges of adjusting to community living.

Sessions are provided in person in the provider's office or the participant's home and are tailored to each participant's goals and Service Plan. CIC has an annual limit of 220 hours, a weekly limit of 4 hours, and is available for up to two years from the initiation date.

A participant may receive both CIC and Peer Mentoring when both needs are documented in their Service Plan. Both services must be tracked independently with separate hour balances, separate session notes, and no overlap in billing records.

CIC Documentation

CIC Documentation Software Built for NHTD Waiver Agencies

Home Health Diary's CIC Software helps NHTD Waiver agencies manage compliance, track service utilization, and maintain accurate counseling records.

Live Annual Hour Balance

When a participant approaches the cap, Home Health Diary alerts your team, giving you time to prepare a medically justified RRDC over-limit request before services are disrupted and your eMedNY billing is affected. The over-limit request, medical justification, and RRDC authorization are all stored in the participant's profile and retrievable instantly.

Weekly Hour Visibility

Home Health Diary shows the current week's CIC hours per participant so coordinators can plan sessions within the 4-hour weekly limit without guesswork. Exceeding the weekly cap creates a billing compliance issue that RRDC reviewers will flag immediately during any audit.

In-Person Session Logging

CIC sessions must be delivered in person in the provider's office or the participant's home. Home Health Diary includes a session delivery field that confirms the in-person location for every CIC note. Telehealth entries are flagged automatically, preventing a documentation error that would trigger RRDC rejection during billing review.

2-Year Service Window Tracking

Home Health Diary tracks each participant's CIC initiation date and sends alerts as they approach the two-year service window. If the window lapses without documentation of medical necessity, the participant loses access to CIC and your agency faces a compliance gap with no warning. Home Health Diary sends alerts at 60, 30, and 15 days before the window closes giving your team time to document medical necessity and request an extension. This is a critical feature no generic community integration services tools platform provides.

CIC + Peer Mentoring Co-Documentation

When a participant receives both CIC and Peer Mentoring, both needs must be documented separately in the Service Plan. Home Health Diary maintains separate organized records for each service within the same participant profile. Both services are linked to the Service Plan and tracked independently against their own hour balances. No duplication. No documentation gaps.

CIC Session Note Writing AI-POWERED

Home Health Diary's built-in AI editor helps CIC counselors write session notes aligned with each participant's community integration goals and Service Plan. The AI understands CIC context emotional adjustment to community living, disability adaptation themes, session progress, participant milestones, and goal updates. As purpose-built CIC documentation software, the AI builds participant context over time recalling their active Service Plan goals, past session themes, and 2-year window status so every new session starts with a full history already loaded.
